Any cause of action or claim you may have against us must be commenced within twelve months after that cause of action or claim arises. The unenforceability of any provision shall not affect the enforceability of the remainder of this Agreement. This Agreement constitutes the entire agreement between Chorus Community Health Plans and you. This Agreement supersedes all prior agreements, representations, proposals related to this website and its content.
