Registering with CCHP’s Provider Portal is the key to accessing the auto-authorization tool and other services. If you're a new provider to the CCHP network or haven't registered for the CCHP Provider Portal yet, your organization's designated site administrator will need to obtain a registration code before they can complete this CCHP online portal registration form.
