Chorus Community Health Plans announces $1.3 million in value-based payouts to network providers
MILWAUKEE, Wis., Oct. 3, 2023 — Chorus Community Health Plans (CCHP), an affiliate of Children’s Wisconsin, has announced that it will provide a total of $1.3 million in value-based care incentive payouts to healthcare providers who participate in the CCHP network. Value-based payouts help support healthcare provider efforts to improve the quality of care by prioritizing access to preventative care for their patients.
“Our partner providers have worked hard to improve healthcare outcomes for their patients, and the results show a promising trajectory,” said Mark Rakowski, CEO of Chorus Community Health Plans. “When health plans work together with their healthcare providers to improve the health of their families and themselves, there is a ripple effect improving the overall health of the community.”
CCHP’s value-based program rewards health care providers with incentive payments for the quality of care they provide to CCHP’s members enrolled in Wisconsin’s BadgerCarePlus Medicaid program. These value-based programs are part of a larger quality strategy to reform how health care is delivered. They are paid for by incentivizing services and related care that results in improved individual and overall community health while reducing unnecessary, high cost emergency related care.
Improvements in healthcare outcomes have been seen across a variety of populations. In pediatric populations there has been an increase in child and adolescent immunizations, lead testing and well-child visits. For maternal fetal health, increases have been seen in prenatal and postpartum care. Finally, positive healthcare outcomes have been measured in asthma management, diabetes, blood pressure and follow-up mental health care.
