We believe health insurance has the power to change lives

Chorus Community Health Plans (CCHP), an affiliate of Children’s Wisconsin, is an insurance organization dedicated to providing our members access to the highest quality health care and services while bringing together community partners and providers to reduce health disparities, improve health outcomes, and design programs that support our members. We customize our services to meet the unique needs of our individual adult, children and family members and help empower them to take charge of their health and wellness. 

CCHP is a Wisconsin health insurance company that offers two products to more than 150,000 Wisconsin members:

  • BadgerCare Plus (Medicaid): CCHP provides health insurance services for more than 150,000 BadgerCare Plus adults and children across southeast and northeast Wisconsin. With the combined resources of Children’s Wisconsin and our community partners, CCHP is committed to improving the health and well-being of our members and the communities we serve.
  • Individual and family plans: CCHP offers on-exchange and off-exchange health insurance plans for individuals and families living in southeast and northeast Wisconsin. Our marketplace and commercial plans offer access to affordable, high-quality health care from a broad network of providers.

We are passionate about expanding access to health care, advancing health equity and building stronger communities where our members live, learn, work and play. We recognize many different factors impact health. That’s why we work together with members, community partners and health care providers to reduce health disparities and design services aimed at improving the health outcomes of our members.

With our commitment to care, we go beyond the standard health plan services and programs. At CCHP, we believe health starts in our communities, our homes, schools and workplaces. CCHP strengthens our communities through strategic partnerships focused on:

  • Increasing access to fresh food and a healthy lifestyle by offering healthy shopping classes at local grocery stores, discounts on fresh produce and free physical activities in safe environments.
  • Offering special transportation options to ensure members receive the care they need.
  • Expanding access to safe and affordable housing with support from Housing First.
  • Establishing employment connections at unique events and workshops with W2 agencies and career development organizations.

CCHP is a member of the Association for Community Affiliated Plans (ACAP), a national trade association representing more than 75 nonprofit safety net health plans that serve more than 22 million low-income individuals in 27 states and the District of Columbia. ACAP’s mission is to represent and strengthen not-for-profit, safety-net health plans as they work in their communities to improve the health and well-being of vulnerable populations.

The National Committee for Quality Assurance (NCQA) has awarded a Commendable status to our BadgerCare Plus product and an Accredited status to our marketplace and commercial plans. NCQA is a private, non-profit organization dedicated to improving health care quality and committed to providing health care quality information for consumers, purchasers and health care providers and researchers. CCHP has also successfully completed the NCQA certified Healthcare Effectiveness Data and Information Set (HEDIS) Compliance audit.
