We are proud to offer an exclusive wellness portal that makes managing and maintaining a healthier lifestyle simple and convenient. Our plans offer a unique portal experience for each plan that you are on, Individual and Family Plans or BadgerCare Plus.
Are you registered for the Wellness Portal?
As a CCHP Medicaid member or Individual and Family plan member you have access to register to the Wellness Portal where you can learn about your health, complete activities and get incentives!
We care about your health and well-being and want to reward you for taking steps toward a healthier lifestyle. Here are few examples of ways that you can earn reward points by completing wellness activities such as:
- Health needs survey*
- Wellness visit*
- Diabetes-related wellness activities*
- Postpartum visit*
Visit your Wellness Portal account to learn about additional wellness activities and to redeem your reward points. Points can be redeemed for either physical or electronic gift cards from a variety of retailers.
Please note that reward points expire 6 months after they are earned. You should regularly check your Wellness Portal account to monitor the status of your points.

As a CCHP member, you have access to register to the Wellness Portal which is an online, interactive tool that you can use to learn more about your health, explore healthy lifestyle choices, view upcoming CCHP classes and community events, and earn rewards for completing wellness activities.
Yes, all members can visit this page to register for the Wellness Portal or click the blue button on this page above that says Wellness Portal.
Visit Chorus Community Health Plan's website https://chorushealthplans.org and click "Our Plans", and then "Health Programs" on the right side of the navigation.
As an Individual and family Plan or BadgerCare Plus Medicaid member visit this page to register for the Wellness Portal.
Individual and Family Plan members will need to know their member ID number and have a valid email address to create a Wellness Portal account. A valid email address is necessary to receive Wellness Portal earned rewards. Your Member ID number is listed on the front of the Chorus insurance card. An example is shown below. If you do not have your insurance card, please call CCHP at 414-266-3636 to obtain your member ID.
As a BadgerCare Plus Medicaid members visit this page to register for the Wellness Portal.
BadgerCare Plus Medicaid members will need to know their 10 digit member ID number and have a valid email address to create a Wellness Portal account. A valid email address is necessary to receive Wellness Portal earned rewards. Member ID numbers are listed on the front of your ForwardHealth insurance card. An example is shown below. If you do not have your insurance card, please call ForwardHealth at 1-800-362-3002 to obtain your member ID.
After you are registered for the Wellness Portal, you will receive alerts in the Wellness Portal message box. These alerts are intended to inform you about important messages, updates, and reminders.

If you are an active CCHP member you can earn reward points for completing wellness activities. Please note that points expire 6 months after they are earned. You should regularly check your Wellness Portal account and your personal email to receive updates about your reward points' status.
The Wellness Portal has an extensive rewards catalog that active CCHP members can browse and select rewards from. Reward points can be redeemed for either physical or electronic gift cards from a variety of vendors.
As an active CCHP member, after you complete your wellness activity, you will receive an email informing you that your rewards points are available. You should click on the link in this email, browse through the rewards catalog and select a gift card(s) of your choice!
If you would like to receive an electronic gift card but do not have access to a smartphone, computer, or printer you can call the Wellness Portal Line at 414-266-3636 to request an electronic gift card to be printed and mailed to you.
No. Only active CCHP members are able to access the Wellness Portal and redeem their incentive(s). This means that if one's healthcare coverage with CCHP ends, that individual will no longer be eligible to redeem their incentive(s). For this reason, you are encouraged to redeem your incentive(s) as soon as they become available.
